THe rational distribution of resources is a key problem in US healthcare - this very basic objective is thwarted by a wide cariety of forces that drive up costs while driving down quality. These forces include a misaligned payment system, the lack of geographic responsibility and accountability, competition for paying patients, exuberance about new technology, poor role definition, and the lack of leadership.
