Abstract: Clinicians frequently manage animal bites and associated risks of rabies exposure. Guidelines regarding wound repair, indications for prophylactic antibiotics, and details for post-exposure rabies prophylaxis regimens are available. Suture repair of bites can be controversial, but generally accepted guidelines suggest that the lowest risks for infection are wounds repaired within eight hours. Clinicians should consider primary wound closure for bites to cosmetically important locations, such as the face, where risks of infection are lower. Wounds older than 24 hours or bites to the hand are at higher risk of infection and may not be good candidates for primary wound closure. The value of prophylactic antibiotics is uncertain, but clinicians may consider treatment for patients with higher risks based on bite location and severity, patient co-morbidities, and bites from humans or cats. Post-exposure rabies prophylaxis is recommended for all wild animal bites. For bites from domestic animals that are available for observation, report incidents to animal control authorities and prophylaxis may not be necessary. For animals that are not available for observation, post-exposure prophylaxis should be initiated. Bat exposures, bat sightings, and the risks of bat rabies can be of great concern to patients and their families. Recommendations regarding variations of potential bat exposures continue to be challenging, and clinicians should be aware of the current guidelines from the Centers for Disease Control for these circumstances. Clinicians can also utilize local and state resources through animal control and public health agencies for additional guidance concerning rabies post-exposure prophylaxis.
