Population Health Working Group

Our working group has been thinking about Population Health, Health Determinants and redefining Health for over ten years. It is an interdisciplinary group whose goal is to create and support local champions of Population Health. We do that by creating tools–like this video, and this blog–to help them exchange knowledge, gather and use evidence, that help them (and us) to use population health strategies in their work.
