CYF - Conferencing and Teaming Overview - 10/2/2014

Recently introduced, Conferencing and Teaming is the DHS practice of engaging individuals, families, supports and professionals to assess strengths and needs and develop a plan for keeping children, teens and adults safe and healthy while continually integrating individual and family actions with professional services. After an initial implementation in the Office of Children, Youth and Families, plans are underway to expand Conferencing and Teaming into other service areas within DHS.
