Johnathon was living in what was called a "Local based hospital unit" he has been in institutions since he was 10 years old until about 5 years ago.
There was a drive to close these institutions down and eventually it was agreed that he should move down to
be nearer his family.
He moved to a "cottage" the only problem was that is was on the junction of two roads one of them the main
A road. There was no garden that could be accessed directly from the property and he was basically
housebound within the four walls.
When the package was commissioned by the local authority on behalf of the NHS the lead carer from the
previous hospital was transferred via a tupe arrangement to the agency that had been commissioned to
provide the care.
The agency was not local to where Jonathon was living and he was being personally charged for a manager to
visit each week to drop off his "Pocket money" etc. There was no flexibility and there were often gaps In the
staffing that were covered by "Agency staff" that did not know Jonathon or his needs. This led to high levels of
anxiety for both Johnathon and the carers as they did not know how to manage him or react to his behaviours.
Eventually the agency gave notice as the package was no longer sustainable. as with a lot of this type of issue
the NHS who by this time were responsible for all Johnathon's funding reaction was increase the money being
offered to the agency to continue to care for him. This they did for a further year which gave time for
alternative provision to be made.
Jonathon's sister's arranged for the management of his personal affairs to be transferred from local authority
administration to their control. In addition they agreed to manage his care and support via a Personal Health
budget. Throughout all this time the main carer from the hospital has continued to be the mainstay of
Johnathon's support but she is retrieving soon.
Several things have happened to improve Johnathon's situation. His family have arranged for him to move to a
bungalow away from any traffic (As you will see it is a mile down a track) The property is not ideal but is it
infinitely better than the previous one.
The is now a stable and local staff team who have all been trained to meet Jonathon's needs. Following the
change in his personal finance management he now has his own motorbility car so that he can get out and
about where previously he was being "confined" by circumstances and risk. He is now a much happier and healthier individual with a much less stressful life.
