Decisions are made based on the data available at the time. Some decisions can be made fairly easy and quick such as deciding what to eat. On the other hand there are many decisions require more thought on the right direction such as the type of mortgage or investments. Senior leadership makes numerous decisions that affect many groups within an organization; some decisions may seem trivial and some may negatively affect individuals. The amount of data required to assist with these decisions is what we need to understand. How much data did they have that caused them to reach that decisions.
Root Cause Analysis is one set of data that affects IT projects. Decisions such as resourcing, budgets, and project selection are sometimes based on how much rework was done on previous projects. Providing a count of defects within specific categories, such as code or requirements, only provides a partial story of what is occurring within an organization. It could also create friction between groups as the subjective and vague nature does not provide the deep understanding to what is truly happening.
