After recalling recent history, the originally foreseen accelerator physic challenges of the LHC are revisited in the light of commissioning experience. After a rocky start, the LHC is now operating above design luminosity and delivering integrated luminosity at an impressive rate. It has also proven to be remarkably flexible and delivers to a wide range of programs which include forward physics and heavy ions. The systems, instrumentation, software, and understanding that have enabled this success are explored. The plans and challenges for the high-luminosity LHC and the accompanying injector upgrades are well established. These are outlined and contrasted with progress so far.
