Community Action Partnership of San Luis Obispo's (CAPSLO) Recuperative Care Program
Effective August 2016, CenCal Health in collaboration with CAPSLO launched the San Luis Obispo County Recuperative Care Program (RCP). The RCP under the supervision of CAPSLO staff , provides an opportunity for CenCal Health members who have little to no resources or are homeless and medically fragile, an opportunity to receive case management, nursing, medication management, and medical necessary transportation services in a shelter setting.
An emphasis is placed on making sure members see their primary care physicians for follow up care and receive preventive services. Additional services include home health visits, health education and collaboration with community social service resources.
The RCP is currently providing services at the Maxine Lewis Memorial Shelter on a 24 hour basis. The current program has availability for six beds however the intent is to grow the program when the new 40 Prado Shelter begins operations in 2018.
