The presentation will describe the challenges to appropriate end of life care for frail older adults with dementia and how these challenges can be addressed by a new model of care called PATH. The PATH process was designed for frail older adults and their caregiver/decision makers to gain an understanding of their overall health status and prognosis, and feel empowered to make decisions which focus on quality of life. The presentation will also review the rapid assessment of frailty and how this can be incorporated into care planning.
