George Cole’s health had been declining for weeks. “I had noticed the weight loss, and I was scared. Believe me; I was nervous because I didn’t know what was going on.”
After losing 25 pounds in two months, George decided it was time to go to the emergency department.
“When I met him in December he had been in the hospital for three days. He didn’t have a primary care provider. He had some pretty outstanding things,” said Lyn Carlson, a community care coordinator with Lee Health.
George was diagnosed with Graves’ disease. Lyn Carlson worked with him to make sure he continued to receive care even after he was discharged from the hospital. “It makes all the difference in the world for people to know there’s someone in the system that knows them,” said Carlson.
The community care outreach program is a free service for patients who qualify to ensure they get the support and tools they need to manage chronic conditions and navigate through the health system.
“There’s a difference between having a phone call from a stranger and meet that person in their home and actually seeing the equipment they have, looking at their medications. There’s not a time limit. We don’t just go for 30 days, we go until the problems have been resolved, or we have connected them to the services they need,” explained Carlson.
Lyn also connected George to the chronic disease program at Lee Health to help him manage his new diagnosis. “She told me she would help me and I believed in her. She gave me motivation, and I got a job. I’m thankful for Miss Lyn for being by my side and for the people that she works around, and giving me the confidence,” said George.
