Due to revised laws with the IRS, we are required to obtain W9 forms for any and all
entities we do business with . The W9 is for the requirements of the requestor.

If you're a sole proprietor,you'll need to just use your name in the "name" box and your
social security number in the "social security" box in Part 1 of the document.
If you're a sole proprietor and you're doing business with our company under a different
name other than your name, please write in Doing Business As, or abbreviated DBA, and
then the name you are doing business as, in the "business" box.
If you are incorporated in a C corp., S corp. or LLC, you'll need to put your business name
in the "Business name" line at the top of the document also stating that if you do business
under a different name with our company, noting DBA or Doing business as...and then the
name you're doing business as. Then below in part 1 of the document we'll need the Employer
Identification number in the appropriate box.
The document then needs to be signed and dated.
