Product Recalls are difficult to prevent, detect, and correct when you are using spreadsheets!

If you have every been part of a Product Recall investigation, you know these are the questions that need to be answered:

• Did we know about the risk and miss it? Or did we not even know about the risk?

• Did we underestimate the chances of it happening? (Occurrence)

• Did we underestimate how bad it could be? (Severity)

• Did we underestimate if we could find it? (Detection)

• Did the supplier follow their process flow and control plan based on the PFMEA?

Too often, the answer to these questions are hidden in spreadsheets that are scattered around the organization.
