The Songwriting Charity have previously used self-administered paper survey forms to collect information from participants in their workshops. In Oct2012, DataFax-compliant forms were developed to collect pre- and post-evaluations of the Bully Beat workshops. DF/Net Research, Inc. generously donated hosting space.
Barcoding, pre-printed IDs and ICR (Intelligent Character Recognition) reduced some of the burden of entering data. As shown in this video, musician Ben O'Sullivan (Director of Programmes at the Songwriting Charity) went one step further, successfully capturing text fields via Voice Recognition (using the onboard dictation facility in Mountain Lion).
