Health navigation is a hot topic in HIV care as we seek to increase the number of clients who are linked to and retained in care. Join us for a national webinar discussing best practices, lessons learned and the challenges and opportunities in HIV care navigation and its goals to improve health equity and decrease disparities by addressing barriers to care.
We will share and discuss best practices on patient/client navigation as a key tool of linkage and retention strategies involving several stages of comprehensive care — health care, mental health, housing and employment services. The webinar will highlight the perspectives of Federal agencies, HIV prevention justice advocates and state-level coordinators working with community health workers and linkage specialists on the frontlines of the domestic epidemic.
