The Center for Connected Health, funded by Partners Healthcare, is headed by Dr. Joseph Kvedar.
The idea is to let patients be connected to their health providers by allowing them the use of simple at-home monitors, such as blood pressure kits, electronically linking their daily results to their doctor's office. We follow patient Sandra Rice, who has high blood pressure and find out how the accountability of this system keeps her on her health regimen to lower her daily numbers.
