  1. In this live demonstration at the New Frontiers 2011 meeting in San Francisco, Dr Binmoeller demonstrates EUS-guided drainage of a large pseudocyst. The cyst is found on endoscopic ultrasound (EUS) to be poorly adherent to the gastric wall. Internal drainage into the stomach carries a high risk of perforation and leakage of contents into the peritoneum. He uses the NAVIX device (Xlumena) to access the pseudocyst from the stomach, which creates a tract large enough for immediate insertion of a fully covered expandable metal stent (Wallflex, Boston Scientific). A double pigtail stent is inserted through the metal stent to prevent stent migration.

  2. Live demonstration at the New Frontiers 2012 meeting in San Francisco. Dr Binmoeller demonstrates the RLUB (‘Retract-Ligate-Unroof-Biopsy’ ) technique for the treatment of a gastrointestinal stromal tumor (GIST)
    Using a 3-pronged anchor, the GIST is retracted and everted, enabling full-thickness ligation. “Unroofing” after ligation exposes the underlying tumor for direct biopsies for immunohistologic diagnosis and determination of the mitotic index.

  4. Live demonstration at the New Frontiers 2011 meeting in San Francisco. Dr Binmoeller demonstrates EUS-guided treatment of a giant gastric varix using a "forward-view" echoendoscope. After puncturing the varix under ultrasound guidance, a 15mm coil is deployed inside the varix, followed by 1 ml of cyanoacrylate glue (Octyl-2-cyanoacrylate). The synthetic "wool" fibers of the coil bind and retain the glue in the varix, which promotes varix obliteration, but also reduces the risk of systemic glue embolization. This demonstration illustrates the value of EUS to optimize endoscopic treatment of gastric varices. Reference: Gastrointest Endosc 2011;74:1019-25

  5. Live demonstration at the New Frontiers 2011 meeting in San Francisco. Dr Binmoeller demonstrates 'single-operator, single-session' diagnosis and treatment of a patient with painless jaundice and weight loss:
    1) Diagnostic EUS revealed a gallbladder mass obstructing the bile duct;
    2) EUS-guided FNA established a diagnosis of cancer;
    3) A self- expandable metal stent (SEMS) was inserted to drain the obstructed bile duct.
