I submit for your consideration the case of a 41 year-old USAF pilot previously diagnosed with ocular histoplasmosis by another retinal specialist while stationed in Honolulu. His right eye has now been stable for over a year, although six interventions (PDT x 2, Avastin x 4) were required to quiet an area of peripapillary choroidal neovascularization. He presented rather unexpectedly on January 5 with new metamorphopsias in his heretofore "normal" left eye. I believe that the findings detailed in the video are classic for extrafoveal central serous retinopathy. My instinct is to consider conventional laser ablation, although I promised this very nice airman (and his ophthalmologist at Kirtland Air Force Base) that I would query an august body of retinal congnoscenti in an attempt to elicit some consensus on the "best" possible therapy. As always, any input would be very much appreciated and apologies in advance for the gratuitous 'Top Gun' thematics.
