  2. The Utah Health Information Network (UHIN) is a claims clearinghouse and Utah’s state-designated health information exchange (HIE). Funded by CHT, UHIN set out to put together a more complete picture of the patient by combining clinical and claims data in their state. Through this pilot project, UHIN has developed an outcomes-based public reporting solution and a concrete set of tools to allow others to do so as well.

    To accomplish this, they combined clinical data contained in the HIE database and X12 messages from claims data suppliers. Special software then transformed the X12 messages and sent them, along with HL7 clinical messages from the HIE, to a Master Patient Index (MPI). Then the data was standardized, de-identified and included in several databases where it could be used by a separate application to perform measure reporting.

  4. To test solutions to several pressing transparency issues, CHT funded three innovation pilots. One of those was led by The Health Collaborative, which used clinical and claims data provided by a Cincinnati-area integrated healthcare system to design a scalable technical solution to integrate claims and clinical information at the patient-level. They collected over 400,000 Continuity of Care Documents (CCDs) from providers. These included a broad set of data types such as laboratory tests (including values), medications, and care plan goals across many types of encounters including hospitalizations, home visits, and office visits. Claims data arrived in flat file format and was transmitted from the health plan to Health Collaborative via sFTP. Health Collaborative matched the clinical to the claims records using IBM’s Master Data Management (MDM) and used popHealth® to import CCDs, electronic clinical quality measures (eCQM), definitions, and to calculate several diabetes care quality measures.
