Underwater endoscopy (UWE)

  1. Dr Binmoeller demonstrates the technique of total underwater colonoscopy in this narrated unedited video. Gas is turned off during scope insertion and withdrawal. The continuous "water exchange" method is used, whereby water contaminated with residue is exchanged for clean water. A small (< 10mm) polyp identified during insertion was removed by UW polypectomy technique using a cold snare. The total procedure time was 27 minutes. The procedure was performed under propofol sedation following prior upper endoscopy.

UWE is a novel method of performing diagnosis and therapy within the gastrointestinal tract using water instead of gas (air, C02). Water filling of the bowel enhances the optics (magnification, higher resolution, 3D effect) and facilitates the detection of precancerous (adenomas) and cancerous growths. Therapeutic resections performed with water immersion have conceptual advantages over conventional methods using gas. Videos posted on this site teach techniques of underwater endosccopy.
