      Messages on bone health are generally aimed at the older generation, who are many times already in decline. Now doctors are setting their sites lower; educating a younger audience. “We see bones, we see skeletons, we see pictures of skeletons and we think it’s like a rock. But if you think about your bone, it’s actually alive,” says Dr. David Eichten, who is an orthopedic surgeon on medical staff of Lee Memorial Health System. Alive and growing - up to a point. “Up until your mid-20s most of the bone mass is accumulated. After that, there’s a steady decline, especially in women. After menopause we see a big drop,” says Dr. Eichten. It’s very important to keep your bones healthy. People with brittle bones are more likely to suffer falls, fractures and arthritis. Turns out you can bulk up while you’re young which helps you stay strong later in life. “A lot of that has to do with exercise and weight-bearing exercise. It’s similar to going to the gym. If you’re not working out your muscles, they’re going to be, they’re going to shrink. It’s the same with your bone and your bone density,” says Dr. Eichten. The American Academy of Orthopedic Surgeons is expressing concern that young people are too sedentary, and it could come back to haunt them. Making this generation weaker than in years past, unless they take action. “It’s important to know that their bone is alive, it’s constantly being remodeled and it’s important for exercise and weight-bearing activity so that your bones aren’t soft,” says Dr. Eichten.           She doesn’t view herself as a trail-blazer, but Shirley Alling was the first in Florida to get the XP knee. “He was looking for the right person, and the right condition in the knee and he chose me,” says Alling. Shirley was active and her right knee wasn’t disfigured by arthritis. So her surgeon thought she was a good candidate for this next-generation implant. One that removes the knee joint but spares the ACL. “Every single knee replacement that gets done we remove the ACL. The newest knee out there is the knee called the Biomet XP knee which does maintain the ACL,” says Dr. Ed Humbert, who is an orthopedic surgeon on medical staff of Lee Memorial Health System. This new breed of artificial joint is configured in a ‘U’ shape, to accommodate the ligament. “The contact of the cement to the bone is much less with this newer XP knee but otherwise you can’t keep the ACL, because the ACL needs this little cutout,” says Dr. Humbert. One of the benefits of keeping the ligament intact is that it allows patients to maintain their stability as they navigate their surroundings. “After the surgery, I had four weeks, a month of therapy. After two weeks, I passed all the tests. All my friends around here are a lot younger than I am, and they couldn’t believe how well I was recuperating,” says Alling. In fact, Shirley is still recovering from the operation. When asked how Shirley is able to handle steps, she answers, “I’m pretty good. I take them one at a time.”ematically “Keeping your ACL will give you more of a kinematically normal acting knee. It’s going to actually bend and flex and extend exactly like your knee does before the surgeries. Which is the goal of every knee replacement,” says Dr. Humbert. This procedure is so new, that long-term success remains to be seen. For now, Shirley is looking forward to kicking up her heels with her naturally functioning knee. “Yes, I’m real happy with it, I can do anything I want to, I can go dancing. I can plant flowers,” says Alling.                 For years, total knee replacement trumped what’s called the partial knee. Replacing the entire joint was considered more reliable than removing a single section. Now for many people less is more. “The first thing is, you’re not taking things; the functioning cartilage either of the kneecap or either on the inside or outside part of the knee. The second is that it’s much less invasive. It can be done through a smaller incision,” says Dr. Fletcher Reynolds, who is an orthopedic surgeon on medical staff of Lee Memorial Health System. Picking a proper partial knee patient is crucial to its success. This surgery is not for everyone. “We break the knee down into three separate areas. There’s an inside part of the knee, an outside part of the knee, and then the area where the kneecap and the femur come together. And those are kind of the three parts. If the arthritis is isolated to either the inside or the outside part of the knee then partial knee can be considered,” says Dr. Reynolds. Limited in size and scope, this surgery is also limited to a specific patient population: those with damage in only one of the three knee compartments. It’s becoming a popular choice for people who want to get rid of pain, but keep everything else. “Their knee feels somewhat more natural because a lot of their knee is retained. Some of the ligaments that we sacrifice with knee replacement surgery are retained as well as the other cartilage surfaces,” says Dr. Reynolds. Many patients eventually need a total replacement if their knee continues to deteriorate. But this smaller surgery allows them to hang on to their joints for as long as possible.
