      This was cancer survivor Bill Stephenson’s first day of therapy. He is working to regain his ability to swallow. “There’s an anxiety factor that you know you wonder ‘why do I have to do this, why am I going to have this thing on my neck?’ and it’s tingling away at me.” As electrodes pulsate his throat muscles, he found a little stress reliever. She was sitting right beside him, her name is Daisy and she’s a therapy dog. “I’ve had patients that are highly anxious when we put the electrical stimulus on them and they won’t let you go up past a certain point cause they’re complaining that it hurts and so forth. She’ll sit in the chair next to them and they pet her and they don’t pay any attention to what we have going on,” says Stacey Brill, a swallowing therapist with Lee Memorial Health System. Stacey is Daisy’s owner. During the day, she shares her with rehab staff and at Gulf Coast Medical Center. “She’ll go into the hospital visit patients that are sick in bed but the staff in the hospital and the staff here in the clinic just get a big kick out of it.” Nationwide, therapy dogs are welcomed by medical professionals who find their patients may require less pain medications and are generally happier. Daisy is not only a friend but a fellow cancer survivor, something that cements the bond between her and the patients. “I saw the scar on her hip and said ‘oh’ you know she’s a cancer survivor too? And I said ‘oh great’. Then she got up and starting licking my leg and from then on I started scratching her and then she came over here and laid down and just laid there. Don’t look at me like that,” says Stephenson. In fact, Daisy overcame cancer twice. The first time it was in her jaw. “That’s what I treat, the head and neck cancer patients and then she herself was diagnosed with that so she kind of has something in common with my patients,” says Stacey. Call it life imitating life. Any way you look at it, Daisy and thousands of therapy dogs like her are giving new mean to the phrase ‘creature comfort’.         The human papillomavirus is not only a much talked about sexually transmitted disease, its also widespread in the adult population. “Its thought that 80% of all sexual active patients have been exposed to high risk HPV at some time in their life,” says Dr. Kevin Fleishman, an obstetrician/gynecologist on the medical staff of Lee Memorial Health System. “It’s really an asymptomatic infection. Most women who have HPV have absolutely no symptoms where as other STDs like herpes certainly has symptoms if there’s an outbreak,” says Dr. Fleishman. The cancers most closely associated with HPV are found in women, in particular cervical cancer, which almost always stems from the virus. “More and more cancers have been shown t be associated with the HPV virus, in addition to what we’ve always known about which is cervical cancer. Other female cancers are associated with the virus as well as anal cancer and certain oral cancers,” says Dr. Fleishman. Sixty-five percent of vaginal cancers are linked to the HPV, along with 50% of vulvar cancers. Ninety-five percent of anal cancers and 60% of oropharyngeal cancers are connected to HPV, although they may also be linked to tobacco and alcohol, which is why doctors find such value in the HPV vaccine. “If we prevent the transmission of the virus, then we can prevent the cancer,” says Dr. Fleishman. The vaccine is recommended for girls and boys between the ages of 9 and 12, older if someone hasn’t had sexual contact. “That’s when the person’s immunity is the strongest so you’ll have a much better response and a much better likelihood that the vaccine will take and protect the patient,” says Dr. Fleishman. Given all population’s high exposure to high risk HPV, there is no way to predict who might go on to develop cancer.           With fair hair and fair skin, Pat Pinner knew he faced a better than fair risk for skin cancer. "One day I found something that didn't look right and I thought that it made sense to go get it checked out." A multi-colored mole with an irregular texture and shape concerned him enough to visit the doctor. "It was my right calf, and it had all the things they say you need to look for," says Pat. A biopsy revealed the worst: it was not only cancer, but melanoma, the most dangerous form of skin caner. "Despite all the things that I saw and all the warnings, I still believed that he would tell me it was nothing," says Pat. It was not what he wanted to hear but oncologist Frank Rodriguez says it may have been his luck day. "If melanoma gets picked up early, usually a surgery is all you need. However as the melanoma grows then the chances it will infiltrate surrounding tissues or even spread to the lymph nodes or even beyond the lymph nodes to other organs goes up," says Dr. Rodriguez, an oncologist on the medical staff of Lee Memorial Health System. Pat underwent a radical resection, which mean going back to the site where the mole was and taking out a wider margin of tissue. "To make sure that there's nothing left behind. Now depending on how deep it is you may be done at that point, but most people would recommend getting what's called a sentinel lymph node, which is the lymph node or the part of the immune system that most likely drains to cells that came from that part of the skin," says Dr. Rodriguez. Pat had the sentinel node removed and checked for cancer. The result was negative. In the ten years since, there's been no recurrence. He considers his doctors lifesavers. "I was fortunate enough to be able to have them diagnose this at an early stage and take care of it, and now it's not an issue," says Pat. "Melanoma is a bad disease and it's one that you can really modify the outcomes if you just find it early," says Dr. Rodriguez. Recognizing it quickly and treating it properly can prevent a mole from becoming a mountain.
